A lead goes cold in five minutes.
Your front desk is with a patient. Threshold answers every call, form and message inside a minute — after hours, on weekends, and while both rooms are full. Then it chases what got away, and rebooks what didn't show.
Hi, it's the assistant at Your practice — I'm an AI. Sorry we missed you. Can I help you book?
yes! first time botox. anything saturday?
Saturday at 10:30 or 1:15 with Dana, NP. First visits start with a consult. Which suits you?
10:30 please
You're booked: Saturday 10:30 with Dana. We'll text a short intake form tomorrow.
Four elements. One leak.
Each one chases the same lost patient: the lead you already paid to generate, then never picked up. Installed one at a time, in the order that pays.
- Week 1Lead ResponseStops the leak tonight
- Week 2RecoveryWins back last month's misses
- Week 3RebookingFills the gaps no-shows leave
- Week 4Intake & ConsentLast, because it carries the most risk
One call in four goes unanswered.
Med spas miss 20–35% of inbound calls. At a $536 average ticket, three a day is six figures a year — before a dollar of new marketing. Move the sliders to your practice.
Revenue from patients who reached you and were never picked up.
Every minute after the first is a discount on a lead you already paid for.
The spend that produced the inquiry is sunk the moment it arrives. What happens in the next four minutes decides whether it was an acquisition cost or a donation.
Missed calls × booking rate × average ticket × 250 operating days. Defaults are published industry averages — a starting point, not a forecast for your practice. The audit replaces them with your own numbers.
We start with the truth about what you already bought.
Most practices we meet already pay for three or four tools nobody ever configured. The audit comes first, and it stands on its own.
One week, one verdict.
We call your practice as a prospective patient, at every hour. We read your booking data, your missed-call log, and every AI subscription on the card. You get a written verdict: what works, what to cancel, and what's leaking.
Week oneResponse first, because it pays for the rest.
Trained on your treatments, your pricing and the way your best coordinator actually speaks. Every handoff to your team is named and logged, never silent. Nothing reaches a patient until you've approved the words.
Weeks two to fourOne thing at a time, in the order that pays.
A new element every two weeks. Response time, recovered bookings and revenue on one page, every Friday. Pause whenever the book is full.
OngoingWhat we will and won't put a model in front of.
A published refusal list is worth more than a feature list. This is the one your clinicians should read first.
- Answer, qualify and book — in your voice, on your rules
- Chase the lead your team never got to
- Capture intake and consent before arrival
- Route anything clinical to a named person, fast
- Show you the number that changed, every week
- Put a model between a patient and a medical decision
- Quote a treatment plan or diagnose over the phone
- Pretend to be a person — callers are told, every time
- Replace your coordinators; we buy back their afternoon
- Hold your data anywhere you can't take it back
Anything clinical goes to a named person.
Medications, contraindications, anything that sounds like a symptom: the model stops, tells the patient who will call, and routes it to the clinician on your roster — logged, timed, and on Friday's page.
Forty practices. One awkward hour.
Every week we call forty aesthetics practices in one metro, at the same inconvenient hour, with the same question about a first tox appointment. We log what happens and publish it on Friday — method first, no practice ever named.
Once a week. The numbers, the method, nothing else.
You're on the list for Friday's log.
- No recording. California and Nevada are all-party consent states. Every outcome is logged by hand.
- No practice named, ever. Metro and category only — in posts, charts and replies.
- Never a patient with a real condition. We ask about availability and price for a common treatment. Nothing clinical.
- Method before findings. Sample, metro, hour, script, and what counts as "answered" — published every time.
Priced in public.
Pausing stops new work. Nothing already live stops running — your phone doesn't go quiet because billing did.
The Audit
One week. A written verdict on every AI dollar you're spending and every lead you're losing. Yours to keep, whatever you do next.
- Mystery-shopped at every hour, including 9pm Sunday
- Your current AI and phone stack, reviewed line by line
- Missed-call and booking-data analysis
- A cancel list and a fix list, in order of payback
Practice
All four elements, installed one at a time in the order that pays, then run and watched.
- Response, Recovery, Rebooking, Intake
- One element shipped every two weeks
- Trained on your voice, treatments and pricing
- Pause anytime — live systems keep running
Group
A fractional AI office for a multi-site group. One voice across every location, one data layer, and one person accountable for all of it.
- Everything in Practice, at every location
- One standard across sites, centrally governed
- Per-location tool sprawl, consolidated
- Reporting ready for diligence
The ones we get asked first.
01Do callers know they're talking to a machine?
Yes, every time, in the first sentence. Practices that hide it get caught, and getting caught costs more than the call was worth. In our experience patients don't mind a machine that answers at 9pm — they mind a voicemail.
02We already pay for an AI receptionist. It isn't working.
That's the most common reason people call us, and it's exactly what the audit is for. Usually the tool is fine and nobody ever taught it what a tox appointment is worth, which calls to escalate, or how your practice actually books. Sometimes the answer is to cancel it. We'll tell you either way.
03What about HIPAA?
We sign a BAA before anything touches patient information. Intake and consent carry the real exposure, so they ship last, behind the elements that don't. Anything clinical routes to a named person at your practice — a model never answers a medical question.
04Which booking systems do you work with?
Boulevard, Zenoti, Mangomint and Aesthetic Record directly. Others we can usually reach; if we can't, the audit says so in week one rather than month three.
05What happens if we pause?
New work stops. Everything already live keeps running, and you pay only the running costs of what's switched on. Turning a live system off is always your decision, never a side effect of billing.
06Who actually does the work?
A small senior team from ADVNC8, the same people through the whole engagement. No outsourcing, no account manager relaying messages. It's why we take four practices at a time.
Start with what you're losing.
One week, $3,500, credited in full if you go further. You'll know exactly which leads you're losing and what they cost — whether or not you hire us to fix it.
Thank you, friend.
We'll reply today to set the audit week for your practice. If it's urgent, studio@advnc8.co reaches the same people.
